Invoice / Update Forms

  • Invoice / update forms (mailed to the registered agent on October 1, 2018 with the cover letter): This file contains 1,709 pages - one invoice / update form for each special district - listed in alphabetical order by the name of the special district. To find a specific invoice, open the pdf file and page down to the correct invoice. For faster results, use the search feature in Adobe Acrobat Reader. Look for the the magnifying glass icon or hold the  'Ctrl' key and 'f' key at the same time. When the search box opens, type the name of your special district or a unique key word in its name and press the enter key. Take care to print only the page you need instead of all pages in the file.

We are committed to ensuring your privacy and the security of your information. The department does not maintain credit card information on our server. All credit card activity is handled by Bank of America Cybersource through a secure external web site.

Your privacy is very important to us. Our policy is to collect no personal information about you when you visit our website, unless you specifically choose to make such information available to us.

If you choose to share personal information, such as by sending a message or filling out an electronic form, please be aware that under Florida law, email addresses are public records. We must save it for a designated period of time to comply with the State of Florida's archiving policies. The information will not be disclosed to third parties or other government agencies, unless required by state or federal law. Florida's public records law requires that all information received in connection with state business be made available to anyone upon request, unless the information is subject to a specific statutory exemption.

When you visit our website, the web server automatically recognizes the internet domain and IP address from which you accessed the site. Information is gathered regarding the volume and timing of access to the site by logging the date, time, and pages you visited while at the site. This is done so we can improve the content of our website. Your personal information, including your personal email address, is not identified.

Refund Policy

Section 215.26, Florida Statutes, states in part: "Applications for refunds as provided in this section shall be filed with the Chief Financial Officer, except as otherwise provided herein, within 3 years after the right to such refund shall have accrued else such right shall be barred." Three years is generally interpreted as meaning three years from the date of payment into the State treasury. The Chief Financial Officer has delegated the authority to accept applications for refund to the unit of State government, which initially collected the money.

If the need for refund arises, please download an Application for Refund from the Florida Department of Financial Services web site (see link below), complete all items, and mail the application to the Florida Department of Economic Opportunity, Bureau of Financial Management, 107 E. Madison Street, MSC 120, Tallahassee, Florida 32399-4124.

Please have the following available:

  1. The Special District Fee Invoice and Update Form.
  2. The total due (amount located on the top right side of the invoice).
  3. The invoice number (located on the top left side of the invoice)
  4. A Visa or MasterCard
  5. The billing address for your Visa or MasterCard

Important Reminders!

Please remember, whether paying online, by paper check, or applying for a zero annual fee, you must:

  1. Review the information on the invoice/update form.
  2. Update it if necessary (strike-through incorrect information and write in the new information directly on the form).
  3. Sign and date it.
  4. Mail it to the address on the invoice (in paragraph at top of the invoice), even if no changes are necessary.

The department uses the information on this form to maintain the Official List of Special Districts Online.
