Tax Forms Available Online

The 1099-G tax forms, “Certain Government Payments,” for calendar year 2012 will be mailed between January 15 and January 31, 2013. If you have moved and have an active forwarding address order on file with the US Postal Service, form 1099-G will be forwarded to your new address. Please allow time for delivery of the form before requesting a duplicate.

Form 1099-G and documentation of benefit repayments for calendar year 2012 provided on form FL UCO-49T will be available as of February 07, 2013. To view or print your tax forms, go to the Weekly Claim Certification page. Once you enter your address, your SSN and PIN, you will have the option to print your tax forms, the 1099-G, which shows the total reemployment assistance payments for the year, and the UCO-49T, which would show the amount repaid to the department due to an overpayment.

You may also request duplicate 1099-G forms for current and past years by e-mail at 1099G@deo.myflorida.com or by phone at 1-800-204-2418. This request must include your name, only the last four numbers of your Social Security number, old and new street or mailing, address current phone number and when appropriate, the specific calendar year requested.

 Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to the department. Instead, contact the department by phone or in writing.
