Tax Forms Available Online
Form 1099-G, “Certain Government Payments”, for calendar year 2013 will be mailed between January 15 and January 31, 2014. If you have moved and have an active forwarding address order on file with the US Postal Service, form 1099-G will be forwarded to the your new address. If you need to change your address, you may do so online at our website. Go to CONNECT Claimant Homepage. Once you have reached the login page follow the instructions to log into Connect. You are now on your “Home Page”. On the right side of the screen click on “View and Maintain Account Information”. You now have five additional links under “View and Maintain Account Information”. Click on “Contact Information”. This will take you to the screen to update your address.
Your 1099-G form also will be available to you in your Connect Inbox and on the screen that lists all prior year’s 1099-Gs. To get a duplicate 1099-G form or form FL UCO-49T (documentation of benefit repayments), you must follow the instructions above to get to your “Home Page”. On the right side of the screen click on the link labelled “My 1099-G and 49Ts”. This will take you to the screen to request duplicate 1099-G forms for the current and past years.
You may also request duplicate 1099-G forms for current and for the past three years by email at 1099G@deo.myflorida.com or by phone at 1-800-204-2418. This request must include your name, only the last four numbers of your Social Security number, old and new street or mailing, address current phone number and when appropriate, the specific calendar year requested. Duplicate 1099’s for the years 2009 thru 2012 are currently available.
Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to the department. Instead, contact the department by phone or in writing.
